Why ADMH?

The healthcare needs of adolescents and adults with developmental disabilities are often neglected as they transition from pediatric services. Individuals may encounter a variety of health, psychosocial and behavioral conditions as they access adult health and wellness services in their community. People are living longer and individuals with disabilities may face aging issues. Their healthcare needs are best served within the model of a Patient Centered Medical Home. The ADMH team manages all care and services—acting as the “hub” for the patient’s healthcare.   

ADMH is a not for profit program established in December 2014 to provide care to an underserved population.
