The healthcare needs of adolescents and adults with developmental disabilities are often neglected as they transition from pediatric to adult services. Individuals may encounter a variety of health, psychosocial, and behavioral conditions as they access adult health and wellness services in their community. People are living longer and individuals with disabilities may face aging issues. Their healthcare needs are best served within the model of a Patient-Centered Medical Home. The ADMH team manages care and services, acting as the “hub” for the patient’s healthcare.
ADMH is a not-for-profit program established in December 2014 to provide care to an underserved population.
At ADMH, our mission is to provide high-quality, personalized healthcare to adults with disabilities. However, we currently have a waiting list of patients in need of our services. To address this urgent demand, we are launching a campaign to raise $5,000 for the expansion of our programs, a campaign to raise $5,000 for our Patient Care and Scholarship programs, and finally we are launching a campaign to raise $5,000 for an ADA-accessible exam table, designed specifically to meet the needs of our patients with disabilities. we believe that every patient deserves to be seen in comfort and dignity.
